Here is how you can obtain a copy of your accident report from the law enforcement agency that reported to the crash scene.

How Can I Obtain a Copy of My Traffic Accident Report from the Illinois State Police?

You may request a crash report from the Illinois State Police online or by mail.

Logon to the Official ISP Crash Report web page, and be prepared to provide the following details from the accident:

  • A driver's first and last name
  • The date of the crash
  • The location of the crash, including the Interstate and County
  • The Agency Crash Report Number (located in the upper-middle portion of the motorist report)

Once the relevant information is entered, you can choose to have the accident report emailed or mailed to you by making the appropriate selection.

There is a $5 crash report fee, and all applicable processing fees must be paid before completing the request.

To request a copy of your crash report in writing, by mail, compose a letter listing the same details that are requested in the online form, including:

  • A driver's first and last name
  • The date of the crash
  • The location of the crash, including the Interstate and County
  • The Agency Crash Report Number (located in the upper-middle portion of the motorist report)

Include a check or money order in the amount of $5.00 for a traffic crash report made payable to “Illinois State Police.” Do not send cash.

You may request a crash report from the Chicago Police Department in person, by mail, or online.

Logon to the Official CPD Crash Report web page, and be prepared to provide the following details from the accident, along with payment totaling $6 ($5 fee for the report, and $1 convenience fee):

  • The driver's name
  • The location of the crash
  • The RD number (report number given by the Police at the scene)

To request an accident report in person, appear at:

Applicable Fees for Requesting a Crash Report In Person or By Mail:

  • .50 cents for each copy of a case report.
  • $5.00 for each copy of a Traffic Crash Report
  • $20.00 for a report generated by an accident reconstruction officer or accident reconstruction team in the case of the traffic crash.

How Can I Obtain a Copy of My Traffic Accident Report from the Cook County Sheriff's Department?

Depending on how you request a police report from the Cook County Sheriff's Office, the fees may range from $5 – $20.

Logon to the Official Cook County Sheriff's Department's Crash Report web page and be prepared to create an account before entering the following details from the accident:

  • The report number provided by the law enforcement official at the scene
  • Name, mailing address, phone number, and email
  • Driver's license number
  • Vehicle license plate number
  • The date/time of the accident
  • Location of the accident
